Updated 29 October 2026. Read the source records directly below. Excel is an optional download. This public index preserves the existing EX001-EX020 references and adds IH-1986 without renumbering the timeline.

Start with the original records

The source entries below identify the document, issuing agency, date, page and limits of the finding. Five government documents have been checked for the specific passages described here. Six exhibit rows point to those documents because EX010 and EX017 share the FY2013 plan. Fourteen legacy references still need review; EX013 is a cross-reference. An unresolved reference is an open verification task, not proof that the underlying evidence does not exist.

PDF page means the page counted from the start of the original file. Printed page means the number printed on the document. The small EA and PFAS downloads are clearly labeled excerpts containing the original cover and selected unaltered pages. Their local PDF page counts differ from the original file's page counts.

What can be checked immediately

1985 workplace conditions, documented in 1986

IH-1986 is the Army Industrial Hygiene Special Study, project 55-92-0400-86, dated 9 May 1986, covering the 27 October-2 November 1985 survey. Original PDF pages 3-4 describe diesel exhaust entering recirculated air, earlier asbestos-coated boiler work without protective controls, solvent cleaning and disposal, and a pesticide investigation. Original PDF pages 9-10 discuss the limits of generator testing and ventilation design. The actual report can be opened from the IH-1986 entry below.

The same report records carbon monoxide and solvent sampling below then-applicable limits and background asbestos sampling below detection. Only one generator operated during measurements; the report did not call that a worst-case condition. Dust analysis for MIREX, Heptachlor, Velpar and Atrazine was pending in the executive summary. That passage does not establish a positive pesticide test. These observations concern particular conditions and dates; they do not calculate every worker's historical dose.

Transformers inside the tunnel and exterior soil are separate questions

EX010 / EX017: the Army FY2013 Kunia Installation Action Plan, original PDF page 11, printed page 9, records that several pad-mounted transformers inside the tunnel complex were drained and removed in 1991, with approximately 15 thirty-year-old transformers removed from service. This is a specific primary-source lead for the interior transformer history.

The same page separately describes possible waste-oil application and PCB/lead hot spots near Building 25, followed by investigation finding no threat at those tested hot spots and recommending no further remedial action there. The interior-transformer paragraph does not specify PCB concentrations or individual exposure. The exterior soil finding should not be substituted for an answer about interior work, and the interior history should not be presented as a positive PCB test.

PFAS records changed over time

EX003: the April 2022 preliminary assessment states no areas of potential interest were identified and did not recommend further PFAS investigation at that stage (original PDF pages 42 and 47; printed pages 31 and 36). Its limitations include historical-record gaps, limited identification of off-post sources and no comprehensive well survey.

EX011: the FY2024 Installation Action Plan, PDF page 12, says the Army determined in 2023 that Kunia, Dillingham and Kipapa would proceed to the PFAS site-inspection phase, which the plan describes as underway. Read the two dated records together. Neither an assessment's initial conclusion nor a later investigation plan alone establishes a positive water sample, universal absence of contamination or an individual's medical causation.

Source register and exhibit crosswalk

IH-1986 - Army Industrial Hygiene Special Study 55-92-0400-86, 9 May 1986

Status: Verified source. Locator: PDF 3-4, 9-10.

1985 survey: diesel exhaust recirculation, past asbestos repair without controls, solvent use/disposal and ventilation limitations.

Limits / next action: CO and solvent measurements below then-applicable limits. Background asbestos below detection. Pesticide analyses pending; no positive result established.

EX001 - VA Kunia public health page

Status: Needs review.

Agency position and veteran guidance.

Limits / next action: Web page is context, not the underlying industrial hygiene or cleanup record. Current contents and historical versions require dated verification.

EX002 - Navy HRSOC Final Environmental Assessment, September 2005

Status: Verified source. Locator: Original PDF 47 and 66; printed 3-9 and 3-28.

Aircraft servicing at Kunia; former diesel UST; asbestos/lead in Building 9; historical transformer oil disposal and potential pesticide residues.

Limits / next action: Linked copy is an excerpt of original pages 1, 47, 66. Potential hazards and site history do not establish an individual's exposure dose or medical causation.

EX003 - Army PFAS Preliminary Assessment, April 2022

Status: Verified source. Locator: Original PDF 42 and 47; printed 31 and 36.

No areas of potential interest identified and no further investigation recommended at that assessment stage; documents record and off-site survey limitations.

Limits / next action: Linked copy is an excerpt of original pages 1, 42, 47. Does not establish universal absence of PFAS. Read with the later FY2024 plan (EX011).

EX004 - DoD PFAS progress report, March 2025

Status: Needs review.

Legacy exhibit reference retained.

Limits / next action: Exact dated document and Kunia-specific passages have not been verified in this repair.

EX005 - Water quality report, 2026

Status: Needs review.

Legacy exhibit reference retained.

Limits / next action: Exact system, reporting year, report, sample location and result require verification. Do not infer historical drinking-water exposure.

EX006 - Navy / NAVIFOR NIOC Pacific history

Status: Needs review.

Legacy exhibit reference retained.

Limits / next action: Exact agency document and page needed before using it for a factual claim.

EX007 - INSCOM historical overview

Status: Needs review.

Legacy exhibit reference retained.

Limits / next action: Exact edition, date and page have not been verified.

EX008 - Hawaii SCR47 / SD1 testimony, 2021

Status: Needs review.

Legislative material may document testimony and concerns.

Limits / next action: Identify each submission and author. Testimony is not automatically an agency finding.

EX009 - Hawaii Department of Health UST records

Status: Needs review.

Legacy record group retained.

Limits / next action: Replace broad repository reference with exact facility, record, date and page.

EX010 - Army FY2013 Kunia Installation Action Plan

Status: Verified source. Locator: PDF 11; printed 9.

1991 tunnel transformer removal; exterior PCB/lead investigation; former diesel UST, contaminated soil, 2005 cap and ongoing monitoring.

Limits / next action: Distinguish interior transformer history from exterior soil investigations. Records follow-up finding of no threat at tested PCB/lead hot spots; no further remedial action recommended there.

EX011 - Army FY2024 Installation Action Plan

Status: Verified source. Locator: PDF 12 and 16.

Page 12 says Army determined in 2023 that Kunia and two other sub-installations would proceed to PFAS site inspection. Page 16 describes FSK-01 diesel investigation and remediation.

Limits / next action: A planned/ongoing investigation is not a positive sample result or an individual medical nexus. Preserve dates when comparing the 2022 assessment.

EX012 - NSA locations/history

Status: Needs review.

Legacy exhibit reference retained.

Limits / next action: Exact document needed. Institutional history alone does not establish contaminant exposure.

EX013 - 2005 EA material repeated in timeline

Status: Cross-reference. Locator: See EX002.

Routes to EX002 for the original EA record.

Limits / next action: Not independent corroboration of EX002.

EX014 - SCR47 committee report

Status: Needs review.

Related legislative source, cross-reference EX008.

Limits / next action: Exact committee report must be verified. Do not count repeated testimony as independent scientific evidence.

EX015 - VA military exposure / garrison health context

Status: Needs review.

Legacy context reference retained.

Limits / next action: Identify the precise page and date. General guidance does not establish Kunia-specific exposure.

EX016 - 2017 agency correspondence

Status: Needs review.

Private correspondence reference retained.

Limits / next action: Original correspondence and permission for public release have not been verified. Private emails are not included.

EX017 - 1991 interior transformer removal

Status: Verified source. Locator: FY2013 IAP: PDF 11; printed 9.

Army plan states several pad-mounted transformers within the tunnel were drained/removed in 1991; approximately 15 thirty-year-old transformers removed from service.

Limits / next action: This paragraph does not specify PCB concentration or document any individual's dose. Same original as EX010; not a second independent source.

EX018 - Eyewitness account of PCB-related work

Status: Needs review.

Personal testimony reference retained.

Limits / next action: Must be identified as testimony, with dates, firsthand basis, consent and source. Not independently verified here; private statements are not included.

EX019 - Pesticide procurement / use records

Status: Needs review.

Legacy record group retained. IH-1986 provides a separate, specific investigation lead.

Limits / next action: Exact procurement records remain unresolved. IH-1986 says dust analyses for MIREX, Heptachlor, Velpar and Atrazine were pending; no positive test established by that passage.

EX020 - Memoir / secondary account

Status: Needs review.

Secondary account reference retained.

Limits / next action: Exact edition and page needed. Distinguish memoir from a government environmental record; no inference of contaminant dose.

How to cite and review this evidence

Use a citation such as: IH-1986, US Army Industrial Hygiene Special Study 55-92-0400-86, 9 May 1986, original PDF page 4, executive-summary paragraph 3f. Name the source, date and page with the particular finding and its limitation. Site-level environmental history, eyewitness testimony, a measured sample, and a medical opinion answer different questions. Keep them identified when combining records.

For an individual review, submit the relevant original pages through the institution's accepted evidence channel, alongside the person's service dates, location, duties and medical evidence. A public link is a reference aid; publication does not prove that an agency received, reviewed or accepted the record.

If a source cannot be opened, record the access failure and request a copy. A failed search or inaccessible attachment does not establish that no evidence exists. Read a primary document before attributing a conclusion to it. Where a source leaves a question unresolved, preserve that uncertainty and identify the next record needed.

Corrections and verification history

29 October 2026: added direct public copies of the 1986 study, FY2013 plan and FY2024 plan; added labeled excerpts of the 2005 EA and 2022 PFAS assessment; supplied page locators and a corrected downloadable index; replaced filename-built Dropbox preview links in this index with confirmed document URLs; made outstanding verification visible. The older spreadsheet is superseded for public navigation. Private medical files, private correspondence and unapproved personal statements are not included in this public register.
